You use it if the item changes the skill in some way (excluding level increase). For example, 
Iris's Form - Wand summons a Nick doppelganger whenever you use Nicholas Rapport, so that skill should be included in skill alteration.
On the item page itself setting it does absolutely nothing, but you'll be able to find the item if you search for the skill using Special:RunQuery/Equipment query or if you set the parameter in Template:Equipment list.
